We are facing Apache crash issue.

Here is the message in error.log :
[Tue May 06 13:03:07.911621 2014] [mpm_winnt:notice] [pid 1084:tid 440] AH00428: Parent: child process 1736 exited with status 255 -- Restarting.

Configuration :
Apache/2.4.9 (Win32) OpenSSL/1.0.1g OpenAM WPA/3.3.0-2 configured
Windows Server 2008 R2 Standard (64 bits)
We load 2 unusual modules : OpenAM is a self-signed module provided by Cap-Gemini and we also load a plugin for Websphere provided by IBM.

This configuration works fine since 16/04/2014 and now we are getting Apache Crash issue.

Windows Event Viewer Logs:

Faulting application name: httpd.exe, version: 2.4.9.0, time stamp: 0x532585f0
Faulting module name: amsdk.dll, version: 0.0.0.0, time stamp: 0x532c68ac
Exception code: 0xc0000005

Faulting process id: 0x6c8
Faulting application start time: 0x01cf6751870c74a2

Some changed since 16/04/2014 ?

The faulting module is amsdk.dll, do not know which non Apache module uses it. Best is to contact the author of the module.

No major changes in environment, For heartbleed vulnerability we have updated apache 2.4.7 to 2.4.9 only. We are using apache as reverse proxy & for single sign-on purpose only.

Our singin component OpenAM Agent Version: 3.3.0-2

What about this things:
Faulting application name: httpd.exe, version: 2.4.9.0, time stamp: 0x532585f0
Faulting application start time: 0x01cf6751870c74a2
Faulting application path: C:\Apache24\bin\httpd.exe

That causes it and is no standard Apache module, so contact the author.
