Which oracle driver has been installed on the servers?

i had similar problems some years ago. The Oracle Instant Client 11 (not 10!) seems to be the most stable version which php_oci8 can handle very well. The only drawback is: oracle 11 driver cannot communicate with oracle databases lower than v10. It wasn't an issue here, but if you still need to connect to oracle 8/9 you need to use the oracle 10 client which causes much faults on our servers.

That is why we recommend to run php over fcgid instead of the module.
